    Kaupunkirakenteen muutos, tarkastelussa Hiukkavaara ja Toppilansalmi

    Tiivistelmä. Kaupungistuminen on maailmanlaajuinen megatrendi, jossa muuttoliike suuntautuu maalta kaupunkeihin. Se saa aikaan muutoksia muuttotappioseuduilla, mutta erityisesti muuttovoittoseuduilla. Kaupungistuminen ei ole stabiili tai stagnaattinen tapahtuma. Se on täynnä alueellista vaihtelua ja muutosta. Kaupungistumisessa on eri vaiheita kaupunkiin muuton initiaalivaiheesta vastakaupungistumiseen ja seutuistumiseen. Tässä tutkielmassa käsittelen kaupunkitilan ja kaupunkirakenteen muutokseen liittyviä käsitteitä ja teorioita Oulun kaupungin kontekstissa. Oulun kaupungin rakentumisessa on huomattavissa muutoksia ja keskinäisiä ristiriitoja, joita käyn läpi Hiukkavaaran ja Toppilansalmen alueita esimerkkeinä käyttäen. Oulun alueella voidaan havaita kaupunkirakenteen hajoamista. Oulun kaupunki laajenee lähiöidensä kautta yhä laajemmalle, mutta samaan aikaan tietyillä asuinalueilla tiivistetään ja tehostetaan rakentamista. Oulussa on huomattavissa eri asuinalueilla seutuistumista ja ’urban sprawl’ tyyppistä kaupunkirakenteen laajenemista. Täydennys- ja uudisrakentamisalueilla lähellä keskustaa on taas huomattavissa vahvasti yksiöpainotteista rakentamista, joka voi vahvistaa muun muassa gentrifikaatiota, studentifikaatiota ja asuntosijoittajapainotteista rakentamista. Tutkielmassa käyn läpi, kuinka kaupunkirakenne on muutoksessa maailmalla ja Suomessa ja kuinka nämä muutoksen vaiheet vaikuttavat alueisiin ja kaupunkeihin. Peilaan yleisiä kaupunkirakenteen muutoksia Oulun kahteen asuinalueeseen Hiukkavaaraan ja Toppilansalmeen ja pohdin näissä asuinalueissa tapahtuvia muutoksia Oulun kaupungin rakenteeseen

    Frequent pain in older people with and without diabetes - Finnish community based study

    Background: The association between pain and diabetes in older people has been largely unexplored. The aim of this survey was to analyze the prevalence and characteristics of pain among Finnish men and women 65 or older with and without diabetes in primary care. Methods: All home-dwelling persons 65 years or older with diabetes (N = 527) and age and gender matched controls (N = 890) were identified from electronic patient records. Frequent pain was regarded as any pain experienced more often than once a week, and it was divided into pain experienced several times a week but not daily and pain experienced daily or continuously. The Numeric Rating Scale (0-10) (NRS) was used to assess the intensity and interference of the pain. Results: The number of subjects who returned the questionnaire was 1084 (76.5%). The prevalence of frequent pain in the preceding week was 50% among women without diabetes and 63% among women with diabetes (adjusted, p = 0.22). In men, the corresponding proportions were 42% without diabetes and 47% with diabetes (adjusted, p = 0.58). In both genders, depressive symptoms and the number of comorbidities were associated with pain experienced more often than once a week and with daily pain. Diabetes was not associated with pain intensity or pain interference in either women or men. Conclusions: Pain in older adults is associated with depressive symptoms and the number of comorbidities more than with diabetes itself.Peer reviewe

    On SAT representations of XOR constraints

    We study the representation of systems S of linear equations over the two-element field (aka xor- or parity-constraints) via conjunctive normal forms F (boolean clause-sets). First we consider the problem of finding an "arc-consistent" representation ("AC"), meaning that unit-clause propagation will fix all forced assignments for all possible instantiations of the xor-variables. Our main negative result is that there is no polysize AC-representation in general. On the positive side we show that finding such an AC-representation is fixed-parameter tractable (fpt) in the number of equations. Then we turn to a stronger criterion of representation, namely propagation completeness ("PC") --- while AC only covers the variables of S, now all the variables in F (the variables in S plus auxiliary variables) are considered for PC. We show that the standard translation actually yields a PC representation for one equation, but fails so for two equations (in fact arbitrarily badly). We show that with a more intelligent translation we can also easily compute a translation to PC for two equations.     Hard Instances of the Constrained Discrete Logarithm Problem

    The discrete logarithm problem (DLP) generalizes to the constrained DLP, where the secret exponent xx belongs to a set known to the attacker. The complexity of generic algorithms for solving the constrained DLP depends on the choice of the set. Motivated by cryptographic applications, we study sets with succinct representation for which the constrained DLP is hard. We draw on earlier results due to Erd\"os et al. and Schnorr, develop geometric tools such as generalized Menelaus' theorem for proving lower bounds on the complexity of the constrained DLP, and construct sets with succinct representation with provable non-trivial lower bounds

    Keinot edistää sää- ja ilmastoriskien hallintaa

    ELASTINEN-tutkimushankkeessa selvitettiin sää- ja ilmastoriskien hallinnan tilaa Suomessa ja arvioitiin riskienhallintakeinoja sekä eri toimijoiden roolia. Lisäksi tarkasteltiin, miten riskienhallinnan kustannuksia ja hyötyjä arvioidaan sekä miten riskienhallinta voidaan kääntää liiketoiminnaksi. Yhteiskunnan toimivuuden ja turvallisuuden ylläpitäminen edellyttää aktiivista varautumista sään ääriilmiöihin. Hankkeen tulosten mukaan suomalaiset organisaatiot eivät usein arvioi sää- ja ilmastoriskejä systemaattisesti. Varautumisessa tulee ottaa huomioon, että ilmastonmuutos voi muuttaa sään ääri-ilmiöitä, niiden esiintymistiheyttä ja voimakkuutta. Näin varautuminen sään ääri ilmiöihin on myös osa sopeutumista ilmastonmuutokseen. ELASTINEN-hanke esittää kolme suositusta, joilla sää- ja ilmastoriskien hallintaa voitaisiin parantaa ja sopeutumistyötä tukea: 1) monipuolistetaan tiedon tuottoa ja käyttöä, 2) vahvistetaan yhteistyötä ja kehitetään toimintatapoja sekä 3) kehitetään palveluita ja liiketoimintamahdollisuuksia. Suositusten toteuttamiseksi esitetään toimenpide-ehdotuksia, minkä lisäksi on tunnistettu toimien toteuttajatahoja. Nykyistä monipuolisempi ja helpommin saatavilla oleva tieto parantaisi mahdollisuuksia arvioida ja hallita sää- ja ilmastoriskejä sekä kykyä sopeutua ilmastonmuutoksen vaikutuksiin. Suomessa tulisi myös arvioida säännöllisesti Suomen ulkopuolella tapahtuvia ilmastonmuutoksen vaikutuksia, jotka voivat heijastua Suomeen. Sää- ja ilmastoriskien hallintatoimia tulisi valita ja arvioida myös taloudellisen tehokkuuden näkökulmasta

    Gabapentin for complex regional pain syndrome in Machado-Joseph disease: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Chronic pain is a common problem for patients with Machado-Joseph disease. Most of the chronic pain in Machado-Joseph disease has been reported to be of musculoskeletal origin, but now there seems to be different chronic pain in patients with Machado-Joseph disease.</p> <p>Case presentation</p> <p>A 29-year-old man (Han Chinese, Hoklo) with Machado-Joseph disease experienced severe chronic pain in both feet, cutaneous thermal change, thermal hypersensitivity, focal edema, and sweating and had a history of bone fracture. These symptoms were compatible with a diagnosis of complex regional pain syndrome. After common analgesics failed to relieve his pain, gabapentin was added and titrated to 2000 mg/day (500 mg every six hours) in less than two weeks. This relieved 40% of his pain and led to significant clinical improvement.</p> <p>Conclusions</p> <p>The pathophysiology of complex regional pain syndrome includes peripheral and central sensitizations, the latter of which might be associated with the neurodegeneration in Machado-Joseph disease. In this report, we suggest that gabapentin could inhibit central sensitization as an adjunct for complex regional pain syndrome in patients with Machado-Joseph disease.</p

    Pain thresholds and intensities of CRPS type I and neuropathic pain in respect to sex

    Abstract Background and aims Healthy women have generally been found to have increased experimental pain perception and chronic pain has a higher prevalence in female as compared to male patients. However, no study has investigated whether pain intensity and pain perception thresholds are distinct or similar between sexes within various chronic pain entities. We investigated whether average pain intensities and pain thresholds assessed using quantitative sensory testing (QST) differed between women and men suffering from three distinct chronic pain conditions: Complex Regional Pain Syndrome (CRPS type I), peripheral nerve injury (PNI) or polyneuropathy (PNP), as compared to paired healthy volunteers. Methods QST data of 1252 patients (669 female, 583 male) with PNI (n=342), PNP (n=571) or CRPS (n=339), and average pain intensity reports from previously published studies were included. Absolute and z-values (adjusted for age and body region) of cold, heat, pressure (PPT) and pinprick pain thresholds were compared in generalized linear models with aetiology, duration of underlying pain disease and average pain intensity as fixed effects. Results Average pain intensity during the past four weeks did not differ between women and men, in both mean and range.     An ARHGAP25 variant links aberrant Rac1 function to early-onset skeletal fragility

    Ras homologous guanosine triphosphatases (RhoGTPases) control several cellular functions, including cytoskeletal actin remodeling and cell migration. Their activities are downregulated by GTPase-activating proteins (GAPs). Although RhoGTPases are implicated in bone remodeling and osteoclast and osteoblast function, their significance in human bone health and disease remains elusive. Here, we report defective RhoGTPase regulation as a cause of severe, early-onset, autosomal-dominant skeletal fragility in a three-generation Finnish family. Affected individuals (n = 13) presented with multiple low-energy peripheral and vertebral fractures despite normal bone mineral density (BMD). Bone histomorphometry suggested reduced bone volume, low surface area covered by osteoblasts and osteoclasts, and low bone turnover. Exome sequencing identified a novel heterozygous missense variant c.652G>A (p.G218R) in ARHGAP25, encoding a GAP for Rho-family GTPase Rac1. Variants in the ARHGAP25 5′ untranslated region (UTR) also associated with BMD and fracture risk in the general population, across multiple genomewide association study (GWAS) meta-analyses (lead variant rs10048745). ARHGAP25 messenger RNA (mRNA) was expressed in macrophage colony-stimulating factor (M-CSF)–stimulated human monocytes and mouse osteoblasts, indicating a possible role for ARHGAP25 in osteoclast and osteoblast differentiation and activity. Studies on subject-derived osteoclasts from peripheral blood mononuclear cells did not reveal robust defects in mature osteoclast formation or resorptive activity. However, analysis of osteosarcoma cells overexpressing the ARHGAP25 G218R-mutant, combined with structural modeling, confirmed that the mutant protein had decreased GAP-activity against Rac1, resulting in elevated Rac1 activity, increased cell spreading, and membrane ruffling. Our findings indicate that mutated ARHGAP25 causes aberrant Rac1 function and consequently abnormal bone metabolism, highlighting the importance of RhoGAP signaling in bone metabolism in familial forms of skeletal fragility and in the general population, and expanding our understanding of the molecular pathways underlying skeletal fragility.</p
